iOS issue with reconciling my profile configuration, my device, and my “device name”

Every time I’ve attempted to set up a NextDNS profile on iOS I have the same issue. I can’t seem to get the configuration order right to properly identify my device with my profile. I always end up with three device logs that show up as “My iPhone” and “iPhone 13” and “Unidentified device.”  I’m confused about the order of the setup, whether it’s best to use the app configuration or the iOS configuration, and how to properly “identify my device” within the iOS configuration. And lastly how to properly link my IP address without it leaking out to all the domains at once and end up with my outgoing traffic getting redirected. I got booted from the app yesterday by either Apple or my ISP because of all the DNS leaks. It seems to me like iOS setup isn’t very user-friendly.
