Router setup

When I go to router DNS settings and change the DNS to nextdns from my setup page of routers(the IPv4 ones with linked IP), Does it follow my nextdns profile or is it just basic nextdns?

    You need to link it to your ip address, otherwise it doesn’t know which profile is yours.   Then make sure your clients use the router's DNS server.   If your public IP changes it will need to be relinked.   You might find it easier to put a NextDNS client on your devices and configure them for your profile.
