Add custom notes to queries in Logs and Allow/Deny lists

When you're playing with queries trying to find the right one to block, you sometimes might go thru so many that it's hard to keep track. So it would be very convenient to be able to add your own notes to see what you should or shouldn't touch next time. Also, it would help to see why exactly you've added a specific query to your Allow/Deny list
