Setting NextDNS up as a vendor so we can subscribe

Hello,
I am a systems analyst for Mendocino County Office of Education. We would like to purchase an educational subscription to trial this service at some of our smaller school sites.
The issue is we are a state entity and because of that we need to set NextDNS up as a vendor in our system before we can allocate funds.
It's a fairly straightforward process , we have a form that needs to be filled out and we'd need a W-9.
Is there someone that can help me with this?
