Android/DOT: This device is using NextDNS with no profile

Starting on October 4th, I started getting ads pop through my browser, and noticed that my analytics showed that no more traffic was flowing through NextDNS.  I've made no changes to my profile/settings, and I was not at my 300,000 requests for the month.  The Setup page has the message, "This device is using NextDNS with no profile".

You'll notice a blip a few days after the 4th, in which I installed the app to try out the DOH method to test that my account still worked.  I've seen a couple of other threads that sound similar to this but haven't seen any resolutions.  Here's the 'test' page when it's not working:
