ISP IP Address is making queries over a VPN

I'm using the private DNS feature on an android while running Mullvad, yet a low but still present amount of queries on the analytics page are originating from my ISP's IP Range. I have made sure on ipleak.net and dnsleaktest.com that this IP isn't showing there, and it isn't. But still this concerns me as even NextDNS should not know this IP, regardless there must be some sort of leak present.

    Have you seen any pattern or timeframe to these “extra” requests? 
    Something like in a short burst or while switching network connection.

      R P M I've been checking on the 30min timeframe throughout the day intermittently, and there always is a very small percentage of requests coming from the IP, something like 500 from the VPN and 10 from the ISP.
