How can I prevent myself from ever changing the DNS on my iPhone?
On my iPhone, I use a Screen Time passcode that is managed by someone else, although I can change my DNS in the settings without a Screen Time passcode. Is there a solution if the DNS setting on the iPhone is not easily changed?

You can use the NextDNS app linked to your NextDNS profile and password protect the app in the same way you do for screen time, managed by someone else.

While yes, you can secure the NextDNS app with its own PIN, Apple still leaves the Device Management setting where DNS can be changed back to "Automatic" unsecured. It would be up to Apple to place the Device Management settings under some kind of security (Screen Time or otherwise), but I will also mention that you're not the first person here to bring this up.
