Root CA install on iPhone not working as instructed in KB

I am trying to install root CA in iPhone 14 pro max, iOS 18.6

When I am going to nextdns.io/ca, it downloads .cer file, instead of downloading a configuration so that I can install it from Settings.

What am I missing?

Root CA is only required when using a block page. Is the block page enabled?

I never install root certificates even for routers etc. Those require root access, if you trust the provider of that certificate. NextDNS uses LetsEncrypt for its own profile created certificates but still I don't trust any provider in the Root authority folder access. This opens up the possibility of MITM attacks or worse.
