Always an issue
On my phone, I constantly get "Private DNS server cannot be accessed"
And looking at ping.nextdns.io, there are constantly tons erroring.
Is this service just bad or is there something else going on?
Why is there no status page to track length or type of errors?

Of course, the service is just bad, but only once in a while someone is complaining about it because the rest of them are too shy to say it and stay with this service for so many years because are masochists.
The way it is configured with hundreds of POP and if one go down you are redirected to another the chances of not working are close to zero. In the over 5 years I use it, since it was in beta, there was no failure even once caused by servers going down. There where other issues here and there with other things...

Looks like a bug with "Block Bypass Methods". Disable nextdns temporarily, goto parental controls and disable the feature. That worked for me
